    Introductory programming: a systematic literature review

    As computing becomes a mainstream discipline embedded in the school curriculum and acts as an enabler for an increasing range of academic disciplines in higher education, the literature on introductory programming is growing. Although there have been several reviews that focus on specific aspects of introductory programming, there has been no broad overview of the literature exploring recent trends across the breadth of introductory programming. This paper is the report of an ITiCSE working group that conducted a systematic review in order to gain an overview of the introductory programming literature. Partitioning the literature into papers addressing the student, teaching, the curriculum, and assessment, we explore trends, highlight advances in knowledge over the past 15 years, and indicate possible directions for future research

    Prospective Observational Study on acute Appendicitis Worldwide (POSAW)

    Background: Acute appendicitis (AA) is the most common surgical disease, and appendectomy is the treatment of choice in the majority of cases. A correct diagnosis is key for decreasing the negative appendectomy rate. The management can become difficult in case of complicated appendicitis. The aim of this study is to describe the worldwide clinical and diagnostic work-up and management of AA in surgical departments.Methods: This prospective multicenter observational study was performed in 116 worldwide surgical departments from 44 countries over a 6-month period (April 1, 2016-September 30, 2016). All consecutive patients admitted to surgical departments with a clinical diagnosis of AA were included in the study.Results: A total of 4282 patients were enrolled in the POSAW study, 1928 (45%) women and 2354 (55%) men, with a median age of 29 years. Nine hundred and seven (21.2%) patients underwent an abdominal CT scan, 1856 (43.3%) patients an US, and 285 (6.7%) patients both CT scan and US. A total of 4097 (95.7%) patients underwent surgery; 1809 (42.2%) underwent open appendectomy and 2215 (51.7%) had laparoscopic appendectomy. One hundred eighty-five (4.3%) patients were managed conservatively. Major complications occurred in 199 patients (4.6%). The overall mortality rate was 0.28%.Conclusions: The results of the present study confirm the clinical value of imaging techniques and prognostic scores. Appendectomy remains the most effective treatment of acute appendicitis. Mortality rate is low.</p

    Global validation of the WSES Sepsis Severity Score for patients with complicated intra-abdominal infections : a prospective multicentre study (WISS Study)

    Background: To validate a new practical Sepsis Severity Score for patients with complicated intra-abdominal infections (cIAIs) including the clinical conditions at the admission (severe sepsis/septic shock), the origin of the cIAIs, the delay in source control, the setting of acquisition and any risk factors such as age and immunosuppression. Methods: The WISS study (WSES cIAIs Score Study) is a multicenter observational study underwent in 132 medical institutions worldwide during a four-month study period (October 2014-February 2015). Four thousand five hundred thirty-three patients with a mean age of 51.2 years (range 18-99) were enrolled in the WISS study. Results: Univariate analysis has shown that all factors that were previously included in the WSES Sepsis Severity Score were highly statistically significant between those who died and those who survived (p <0.0001). The multivariate logistic regression model was highly significant (p <0.0001, R-2 = 0.54) and showed that all these factors were independent in predicting mortality of sepsis. Receiver Operator Curve has shown that the WSES Severity Sepsis Score had an excellent prediction for mortality. A score above 5.5 was the best predictor of mortality having a sensitivity of 89.2 %, a specificity of 83.5 % and a positive likelihood ratio of 5.4. Conclusions: WSES Sepsis Severity Score for patients with complicated Intra-abdominal infections can be used on global level. It has shown high sensitivity, specificity, and likelihood ratio that may help us in making clinical decisions.Peer reviewe

    The role of the expression of estrogen receptors in malignant melanomas and their sentinel lymph nodes; association with prognosis and overall survival

    Background: Steroid hormones seem to affect the biological behavior of melanoma. Prior to the discovery of estrogen receptor beta (ERb), melano-mas were considered to contain low estrogen receptor concentrations. Fur-thermore, no studies have examined the role of estrogen receptor expression in sentinel lymph nodes (SLNs) of melanomas. The purpose of this study was to investigate the immunoexpression of estrogen receptors in malignant mel-anomas and SLNs and to examine any possible association with prognosis and overall survival.Patients and methods: A retrospective analysis of prospectively collected data was conducted during a 12-year period (2001-2012). Sixty patients with mean age of 54.4 ± 14.5 years old diagnosed with melanomas after excision biopsy of pre-existing melanocytic lesions, were included in the study. All pa-tients underwent wide excision of the primary tumor and SLN identification. Therapeutic lymph node dissection was conducted in cases where the final pathological report was indicative of SLN tumor invasion. Determination of es-trogen receptor alpha (ERa) and beta (ERb) status by immunohistochemistry on tumor and nodal paraffin blocks was performed in all feasible cases.Results: ERb but not ERa was the predominant estrogen receptor found in all primary tumors and SLNs examined. The most intense ERb immunostaining was seen in negative SLNs associated with thinner, less invading melanomas. ERb expression in the primary tumor seems to correlate with the cellular mi-croenvironment, possibly altering the process of SLN invasion.Conclusions: ERb expression is down-regulated in aggressive melanomas with sentinel nodal disease, suggesting its possible usefulness as a surrogate marker for metastatic potential and prognosis in malignant melanoma.Οι φυλετικές ορμόνες επηρεάζουν τη βιολογική συμπεριφορά του μελανώματος. Μετά την ανακάλυψη των οιστρογονικών υποδοχέων (ER) και ιδίως του υποδοχέα βήτα, η άποψη πως τα μελανώματα εκφράζουν χαμηλές συγκεντρώσεις ER αναθεωρήθηκε. Ο σκοπός της παρούσας μελέτης ήταν να διερευνήσει το βαθμό έκφρασης των ER στο πρωτοπαθές μελάνωμα αλλά και στο λεμφαδένα φρουρό αυτού που παριστά ένα σημαντικό σταθμό στη μεταστατική διαδικασία και να εκτιμήσει πιθανή συσχέτιση με την πρόγνωση και τη συνολική επιβίωση.Μέθοδος. Πραγματοποιήθηκε ανάλυση δεδομένων 60 ασθενών, μέσης ηλικίας 54,4 ± 14,5 ετών, με διάγνωση μελανώματος κατά την περίοδο 2001-2012. Όλοι οι ασθενείς υποβλήθηκαν σε βιοψία λεμφαδένα φρουρού, μετά την αναγνώρισή του με ραδιοϊσοτοπική λεμφαγγειογραφία. Εάν η ιστοπαθολογική εξέταση αναδείκνυε διήθηση του λεμφαδένα από το νεόπλασμα, διενεργείτο συμπληρωματικός λεμφαδενικός καθαρισμός, σε δεύτερο χρόνο. Τα επίπεδα έκφρασης του ERβ είναι ελαττωμένα σταθερά σε επιθετικά μελανώματα, μεγάλου πάχους με μεταστατική νόσο στο λεμφαδένα φρουρό. Η αξιολόγησή τους υποδεικνύει έναν πιθανό δείκτη του μεταστατικού δυναμικού των μελανωμάτων και κατ’ επέκταση της πρόγνωσης της νόσου

    Interoperability-Enhanced Knowledge Management in Law Enforcement: An Integrated Data-Driven Forensic Ontological Approach to Crime Scene Analysis

    Nowadays, more and more sciences are involved in strengthening the work of law enforcement authorities. Scientific documentation is evidence highly respected by the courts in administering justice. As the involvement of science in solving crimes increases, so does human subjectivism, which often leads to wrong conclusions and, consequently, to bad judgments. From the above arises the need to create a single information system that will be fed with scientific evidence such as fingerprints, genetic material, digital data, forensic photographs, information from the forensic report, etc., and also investigative data such as information from witnesses’ statements, the apology of the accused, etc., from various crime scenes that will be able, through formal reasoning procedure, to conclude possible perpetrators. The present study examines a proposal for developing an information system that can be a basis for creating a forensic ontology—a semantic representation of the crime scene—through descriptive logic in the owl semantic language. The Interoperability-Enhanced information system to be developed could assist law enforcement authorities in solving crimes. At the same time, it would promote closer cooperation between academia, civil society, and state institutions by fostering a culture of engagement for the common good

    Semantic Representation of the Intersection of Criminal Law & Civil Tort

    The more complex and globalized social structures become, the greater the need for new ways of exchanging information and knowledge. Legal science is a field that needs to be codified to allow the interoperability between people and states, as well as between humans and machines. The objective of this work is to develop an ontology in order to describe two different pillars of codified law (civil and criminal) and be able to depict the interaction between them. To answer the above question, we examine the Greek Criminal Law as depicted in the Greek Penal Code (ΠΚ) and the way its articles can be analyzed. Then we examine Tort as described in the Greek Civil Code (AΚ) and link the two codifications through the concepts of illegality and damage, both being prerequisites of tortious liability. Following that, through the Protégé application, a legal ontology is created in the OWL semantic language, while finally, four articles of the Penal Code are codified in the ontology and a presentation of their relation to the civil tort is required from a reasoning algorithm

    Neutrophil Gelatinase-Associated Lipocalin as a Biomarker of Acute Kidney Injury in Patients with Morbid Obesity Who Underwent Bariatric Surgery

    Introduction: Neutrophil gelatinase-associated lipocalin (NGAL) has been identified as a biomarker of acute kidney injury (AKI) that could contribute to early diagnosis and effective management of AKI. The purpose of this study was to evaluate NGAL as a predictive marker of AKI in patients with clinically severe obesity (BMI >50) who underwent biliopancreatic diversion surgery. Patients and Methods: We prospectively studied 23 patients with clinically severe obesity who underwent biliopancreatic bypass surgery. NGAL was measured using chemiluminescent microparticle immunoassay in three urine samples collected from each patient before surgery (first sample), 2-6 h after surgery (second sample) and on the first postoperative day (third sample). Results: Renal function was evaluated using serum creatinine values, which were 0.91 ± 0.26 mg/dl before surgery, 0.87 ± 0.31 mg/dl immediately after surgery and 0.92 ± 0.62 mg/dl on the fifth postoperative day. During the immediate postoperative period, AKI was observed in 2 patients, one of whom required renal replacement therapy with hemodialysis. In both patients, urine NGAL increased within the first postoperative hours before the values of serum creatinine increased. Conclusion: Urine NGAL in patients with clinically severe obesity, which was surgically treated, might be a potential biomarker of early AKI detection. Further research is required in order to confirm these results observed in a small number of patients who developed postoperative AKI and make recommendations for predictive NGAL values in patients who underwent bariatric surgery
